New Patient Registration
If you are wanting to register at the practice during this time, we ask if you have recently moved to the area to please first check you are in our practice boundary. You can do this by contacting the surgery to check before completing the registration form. If you do not live within the practice boundary the practice will not be able to accept your registration application.
If you are not moving house outside the boundary area of your current registered practice you are advised to remain registered with your current practice to ensure that you can continue to receive care from a healthcare professional who has immediate access to your full medical records until further notice.
Please let us know what pharmacy you would like any medication to be sent electronically to, at this time we are unable to have anyone collect any prescriptions from the practice. We will supply you with online booking logins so you are able to request your medication online, please let us know an email address you would like to use to enable us to arrange this.
At this time we will not be asking for photo id or proof of address. Please contact us if you need any further information.
