Phoenix Family Practice aims to maintain consistently high levels of health care service, but we appreciate on occasions we may fall short of that mark. In some instances making a complaint is appropriate, and we welcome feedback.
We ask any complaints are provided in writing. We will acknowledge receipt within 3 working days, investigate any issues or concerns raised and provide a response with 28 working days.
In the event you're not happy with the response we've provided, or if you would prefer not complain to the practice please use one of the services below to complain to.
