How can you order repeat prescriptions?
Please complete a form for yourself or on behalf of a friend or relative:
These forms are open 07:00 - 15:00 Monday to Friday, excluding bank holidays
Alternatively, you can request a repeat prescription if you have been prescribed this medication for long term use by a medical practitioner. You will not be able to order medication which you have had previously but have not agreed long term use with your practitioner.
- Requests must be in writing delivered to the surgery or via the online services (i.e. NHS app or above forms).
- If you would like your prescription to be sent electronically to your nominated pharmacy please specify the pharmacy in your request.
Please make sure you allow sufficient time for the prescription to be prepared. This can take two working days from when we receive your request.
Please remember that if you are ordering your repeat prescription early to include the reason why, otherwise it may not be issued.
