Church Street Practice is currently located in modular buildings at 22 Church Street, Weybridge KT13 8DW following the fire on 11th July 2017 which destroyed the previous Primary Care Centre and Hospital. That building had been built in 1999 on the site of the old cottage hospital, and was owned by NHS Property Services Ltd.
We look after the health care needs of approximately 13,000 patients. We share the premises with another medical practice, The Rowan Practice.
A new building will be built on the same site which will house the two Practices, and other services. North West Surrey Clinical Commissioning Group are managing this process and have undertaken to keep the local community informed of progress.
Our philosophy is to provide high quality care to all our patients. We value the traditions of whole family medicine which has always been fundamental in our long established practice. Do you need information from us in a different format – for example, large print or easy read? Do you need help to communicate with us – for example, because you use British Sign Language? The practice has fully implemented and conformed to the Accessible Information Standard.

National Patient Survey
Many of our patients will receive a questionnaire through the post asking about their experiences of local health services including accessing GP services, patient satisfaction with practice opening hours and overall care, the surgery environment and helpfulness of staff. This is a national survey conducted by the Department of Health who has commissioned MORI to send out the questionnaires and analyse the responses.
